The Life Long Care program supports urban Indigenous seniors and Elders, and people living with chronic illness, cognitive impairment, physical disability, or frailty. The program provides transportation services; opportunities for socialization; service navigation and advocacy; and emotional and spiritual supports to promote independent living and good quality of life.
